Location: Multiple locations in New York State
Level 2 trauma Nurse Case Manager needed for acute care hospital to do hospital transition of care discharge planning.
Hospital case management and discharge planning.
Profiles must detail/be clear in regards to discharge planning Internal floating within the department.
Required:
1-2 Years Acute Care Discharge Planning Experience
Preferred: Hospital Transition of Care Discharge Planning
1-2 Years in Acute Care Discharge Planning
