Job Summary
We are seeking a dedicated and experienced Patient Services Manager to oversee patient care coordination within our healthcare facility. The ideal candidate will possess strong leadership skills, extensive knowledge in nutrition and physiology, and a commitment to delivering exceptional patient-centered services. This role involves supervising clinical staff, managing care plans, and ensuring compliance with healthcare standards to promote optimal patient outcomes across various settings including acute care, long-term care, and geriatrics.
Responsibilities
- Lead and supervise the patient services team, ensuring efficient delivery of care and adherence to protocols
- Develop, implement, and monitor individualized care plans tailored to patient needs
- Conduct comprehensive patient assessments, including diagnostic evaluations and nutritional assessments
- Oversee nutrition services, including dietetics, weight loss programs, and tube feeding management
- Coordinate with multidisciplinary teams to facilitate seamless patient care across departments such as dietary, medical, and nursing
- Utilize EMR systems for accurate documentation of patient information, treatment plans, and progress notes
- Provide health coaching to patients to support lifestyle modifications and long-term health goals
- Ensure compliance with healthcare regulations related to hospital experience, acute care, geriatrics, and long-term care settings
- Manage patient education initiatives focused on nutrition, disease management, and wellness strategies
- Monitor quality assurance metrics and implement continuous improvement strategies
Skills
- Proven supervising experience within healthcare or dietary departments
- Extensive knowledge of physiology, nutrition, dietetics, and diagnostic evaluation techniques
- Experience with tube feeding management and developing care plans for complex nutritional needs
- Strong background in health coaching with an emphasis on weight loss and lifestyle changes
- Familiarity with EMR systems for documentation and patient record management
- Prior hospital experience in acute care or long-term care environments is highly desirable
- Knowledge of geriatrics-specific care practices and patient assessment methodologies
- Excellent leadership skills with the ability to supervise multidisciplinary teams effectively
- Strong organizational skills with attention to detail in managing multiple priorities simultaneously
