Job Summary

Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs.
- Develop and implement individualized care plans aligned with best practices in hospital medicine, emergency medicine, and specialty areas such as ICU, PICU, or Geriatrics.
- Coordinate inpatient and outpatient services, ensuring smooth transitions from hospital to post-acute or community settings through effective discharge planning.
- Manage utilization review and management processes by evaluating diagnostic evaluations, medical records, and treatment appropriateness to optimize resource use.
- Collaborate with multidisciplinary teams—including physicians, therapists, social workers, and managed care providers—to deliver holistic patient care.
- Facilitate patient intake procedures and monitor ongoing patient service needs within hospital or clinic environments.
- Ensure compliance with HIPAA regulations and maintain accurate documentation of medical records and case management activities.
- Participate in quality improvement initiatives by analyzing patient outcomes related to infectious disease care, trauma cases from Level I/II trauma centers, or behavioral health interventions.
Qualifications
- Valid RN license with current state registration; additional certifications in case management or specialty areas are a plus.
- Extensive hospital experience across various units such as ICU, ER, Medical-Surgical wards, or Geriatrics; trauma center experience highly preferred.
- Critical care experience including Level II trauma center or Level I Trauma Center exposure is highly desirable.
- Strong knowledge of physiology, medical terminology, diagnostic evaluation techniques, and acute care protocols.
- Proven ability to perform comprehensive patient assessments and develop effective discharge plans in inpatient or outpatient settings.
- Experience with utilization management systems and familiarity with managed care principles.
- Excellent communication skills for patient service delivery and interdisciplinary collaboration.
