Job Summary:
SUNY Upstate Cancer Center is seeking a highly skilled and motivated Advanced Practice Provider to join our Multidisciplinary Palliative Ambulatory Care Team.
Key Responsibilities:
- Seeing appropriate follow-up patients, monitoring patients receiving both IV and oral chemotherapy agents and on clinical trials as well as ensuring efficient care for symptom management of both disease and treatment modalities.
- Conduct collaborative, systematic, ongoing and criterion-based evaluation of the outcomes and continue to assess to revise the diagnosis, outcomes, plan, and implementation as needed.
- Evaluates the accuracy of the diagnosis and the effectiveness of interventions and adapt the plan of care as appropriate for patients.
- Communicates with leadership the patient, family, and system during all transitions of the patients care.
The APP is a member of the multidisciplinary healthcare team who demonstrates professionalism and leadership within the institution and exemplifies the Upstate Mission, Vision and Values and acts in accordance with Upstate policies and procedures.
Minimum Qualifications:
- Master’s Degree as a Nurse Practitioner or Physician Assistant (or graduated from an accredited PA program) or will be eligible by time of appointment.
- NYS Licensed/Registered NP or PA.
- Current National Certification consistent to the clinical practice.
- NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
- CPR-BLS Certification required at the time of appointment.
- Must successfully pass pre-employment and credentialing clearances.
Preferred Qualifications:
Prior experience in specialty area, Oncology preferred.
