Job Summary:
This pharmacist will interpret medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. This includes preparation of compounds, including injectable products, and dispensing of medications for inpatient and ambulatory care use. They will perform duties in all areas of Pharmacy to include inpatient, ambulatory care and satellites including: Provides information to health care practitioners to promote the rational use of medications. Provides drug monitoring service. Maintains Pharmacy records by law and institutional policy. Interprets policy and procedures for hospital staff. Participates in teaching programs for hospital staff. Participates in Pharmacy Departments Quality Assurance Programs. This pharmacist will also provide dedicated patient care and could be assigned to cover the Emergency Department and work within the hospital to provide rapid response and code response. They may also be assigned to complete transitions of care duties including obtaining medication histories, and having oversight for safe and efficient transitions of care from hospital admission to hospital discharge.
Minimum Qualifications:
B.S. in Pharmacy or Pharm.D., NYS license and registration - or eligible - required
Preferred Qualifications:
Completion of PGY1 Pharmacy residency or equivalent hospital/emergency room or transitions of care experience preferred.
Work Days:
Overnights, with 7 on 7 off scheduling. Rotating shifts as necessary for the department.
