Job Summary:
The HELP Nurse Principal Advancement Leader is a professional registered nurse who serves as a leadership resource on the Hospitalist Escalation Team, working closely with multidisciplinary teams including patients and families, physicians, consultants, nursing, ancillary services, social work, case management, and ambulatory services. The role is responsible for facilitating strong collaboration across the care continuum, improving utilization of services, strengthening continuity between inpatient and outpatient settings, and advancing quality outcomes with an additional focus on appropriate length of stay.
The role participates in daily rounding, expedites testing, consultations, and interventions, ensuring scheduling and execution occur reliably to prevent discharge delays. In partnership with department and ambulatory leaders, the role develops multidisciplinary care pathways that are clearly defined, reliable, and sustainable, supporting a shared vision of high‑quality care progression that enhances patient outcomes and responsibly utilizes resources.
The HELP Nurse Principal Advancement Leader must be informed of services and able to identify and recommend appropriate, augmented outpatient follow‑up resources when inpatient care is no longer required. Success in this role requires consistent follow‑through, creative problem‑solving, excellent customer service, impeccable communication, and a commitment to quality, collaboration, continual improvement, and innovation.
Minimum Qualifications:
NYS licensed RN (or eligible), Bachelor of Science in Nursing required, with a minimum of three years of relevant experience, including at least (1) one year in an acute care setting.
Preferred Qualifications:
Master's degree preferred. Previous care transition/case management preferred. Leadership experience
