two full-time RN Case Managers to support individuals with intellectual and developmental disabilities (IDD) in the Kingston, NY area.

RN Case Manager Responsibilities:
- Provide comprehensive nursing case management for individuals with IDD
- Develop, implement, and monitor individualized care plans
- Coordinate services across interdisciplinary teams and outside providers
- Ensure compliance with care standards, documentation, and reporting requirements
- Serve as a primary point of contact for families, providers, and support staff
RN Case Manager Qualifications:
- Current RN license in New York State
- Graduate of an accredited School of Nursing
- At least 3 years of experience working with the IDD population preferred
- Bachelor’s degree in Nursing or related field preferred
- Strong communication, organizational, and care planning skills
