We’re looking for a compassionate and experienced Nurse Practitioner to join our team, delivering comprehensive primary care services to residents of assisted living facilities. If you thrive in a collaborative environment and are committed to high-quality, patient-centered care - apply today!

Key Responsibilities

Perform comprehensive assessments and manage chronic and acute conditions
Develop individualized care plans in collaboration with facility teams
Prescribe medications, order diagnostic tests, and interpret results
Educating patients and families on health maintenance and disease prevention
Participating in chronic care management and quality improvement initiatives
Document all care accurately in our electronic medical record (eCW)

Qualifications

Master’s degree in nursing from an accredited NP program
Active, unrestricted New York State NP license
DEA license (or eligibility)
Medicare/Medicaid provider eligibility

Preferred:

3+ years’ experience in geriatrics or long-term care
Familiarity with eClinicalWorks (eCW)
